About the service

Our Shelley Rise service provides accommodation-based support to two people at any one time, helping them to develop their capacity to live independently and ultimately move on to more permanent independent accommodation. Some of the areas the service can help with are: engaging in employment, education and/or training, maintaining tenancies, managing mental health and relationships and accessing other services as required.

The service is available to adult Hampshire residents 18 years and over with eligible mental health related social care needs. The service is also available to those at risk of deteriorating mental health who do not currently meet the eligibility criteria but would likely to do so if they do not receive a preventative service.

The support provided is person-centred and outcome focussed – service users are involved in any plans regarding changes to the service, and their needs are at the centre of service development and delivery. Individual support plans are jointly developed, and take into consideration the recovery star and wrap planning approach, with a comprehensive risk management plan for each individual.

This is a stage 2 service, which means that individuals receive 2-5 hours of support per week for a period of 6-24 months. Support is provided to plan for and secure a positive move on from the service.
